I understand that while Rootophia Pet Services will take every reasonable precaution to care for my dog safely, dogs are animals and can be unpredictable. I accept that minor injuries, illness, stress, escape attempts, play-related incidents, or dog-to-dog altercations may occasionally occur despite appropriate handling and supervision.
I confirm that I have disclosed all relevant behavioural, medical, and welfare information about my dog. I understand that failure to disclose important information may affect Rootophia Pet Services' ability to care for my dog safely.
I agree that Rootophia Pet Services may refuse or stop services if my dog presents a welfare, safety, behavioural, or handling risk that cannot be reasonably managed.
I agree that if my dog becomes ill or is injured while in Rootophia Pet Services' care, reasonable efforts will be made to contact me or my emergency contact. If this is not possible, I authorise Rootophia Pet Services to seek veterinary advice or treatment as necessary, and I agree to pay any associated costs.
I understand that dogs may interact with other dogs and environments outside of Rootophia Pet Services' direct control during some services, and although every effort is made to reduce risk, incidents cannot always be completely prevented.
