I confirm that the information provided is accurate to the best of my knowledge. I agree to tell Rootophia Groom about relevant health, skin, coat and behavioural concerns. I understand that all displayed grooming prices are starting prices. I understand that the final price can change according to breed, coat condition, matting, behaviour and time required. I understand that additional grooming time is charged at £10 per 15 minutes. I accept the humane de-matting policy and consent to a shorter clip where this is the safest and kindest option. I accept the flea and tick policy and understand that suitable flea and tick shampoo may be used if parasites are discovered. I understand that Rootophia Groom may alter or stop a groom if continuing would compromise welfare or safety. I understand that my appointment is confirmed only when Rootophia Groom has agreed it with me directly. I consent to Rootophia Groom securely using the submitted information to assess and manage my dog's grooming appointment.
